　　年轻干练的米兰达（海莉·贝瑞 Halle Berry 饰）是一名卓有成就的犯罪心理学家，她和丈夫共同经营一家精神治疗机构，两人无论生活还是事业都春风得意，顺风顺水。不过一向理性的米兰达也遇到了难缠的对手，她的病人之一克罗伊（佩内洛普·克鲁兹 Penélope Cruz 饰）因谋害继父背叛入狱，克罗伊经常沉浸在不切实际的幻想中，这令米兰达困扰不已。在某个雨夜，米兰达驱车回家途中险些撞上一名小女孩，继而发生了令人难以置信的事情。当她从昏迷中苏醒后，发现自己被关在了精神病院，接下来则要面对谋杀丈夫的指控。米兰达想方设法要证明自己的清白，在这一过程中也发现了恐怖、黑暗的真相……
